The MOHLTC established a care coordination service on May 31, 2017. In addition to accessing information about MAiD, patients and clinicians can access information/supports for other end-of-life options, such as hospice and palliative care. The care coordination service can also assist patient and care providers with a connection to a MD or NP that can provide MAiD.
